PCFC VOLUNTEER LIABILITY WAIVER & PHOTO RELEASE

I understand that my participation in volunteer activities organized by the Portland Community Football Club (PCFC) (the "Organization") involves certain risks and potential hazards. In consideration of being allowed to participate as a volunteer, I hereby acknowledge and agree to the following terms and conditions:

  1. Assumption of Risk: I acknowledge that my volunteer work may involve physical activities and potential exposure to various risks and hazards, including but not limited to accidents, injuries, property damage, illness, and other unforeseen events. I voluntarily assume all such risks and responsibilities associated with my participation in volunteer activities.

  2. Health and Safety: I certify that I am in good physical condition and have no medical conditions that would prevent me from participating in the volunteer activities. I agree to comply with all safety instructions provided by the Organization and its representatives during my volunteer work.

  3. Release and Waiver: In consideration of being permitted to participate as a volunteer, I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby release, discharge, and hold harmless the Organization, its directors, officers, employees, agents, and volunteers from any and all claims, liabilities, damages, demands, actions, causes of action, costs, and expenses (including legal fees) arising out of or in connection with my participation in the volunteer activities, whether caused by the negligence of the Organization or otherwise.

  4. Confidentiality: I understand that all information concerning players and families, our staff, volunteers, financial data, and the organization’s records is confidential. “Confidential” means that you are free to talk about the Organization and about your position, but you are not permitted to disclose players' or family’s names or talk about them in ways that will make their identity known. No Organization’s name, voice, image, appearance, information, or likeness may be released without appropriate authorization. This is a basic component of player and family care and organizational ethics. We rely on volunteers to conform to this rule of confidentiality to keep our communities, players, and families safe. General information, policy statements, or statistical material that is not identified with any individual or family is not classified as confidential. Staff members are responsible for maintaining the confidentiality of information.

  5. Media Release: I grant the Organization the irrevocable right to use and publish any photographs, videos, or other media taken during my participation in volunteer activities for promotional, educational, or other related purposes without compensation or additional consent.

  6. Governing Law and Severability: This Volunteer Waiver and Release Form shall be governed by and construed in accordance with the laws of Oregon State and the United States of America. If any provision of this agreement is deemed invalid or unenforceable, the remaining provisions shall continue in full force and effect.

    1. Failure to comply. Failure to maintain confidentiality may result in termination or other corrective action. This policy is intended to protect you as well as PCFC because in extreme cases, violations of this policy also may result in personal liability.

I have read this Volunteer Waiver and Media Release Form and fully understand its contents. I acknowledge that I am signing this document voluntarily and without any undue pressure or influence, and confirm that my submission is indicated on my PCFC Volunteer Registration agreement.
